To assist with developing the topics for a Collaborative, an Expert Reference Panel (ERP) is established. The ERP consists of people with research and expertise in a particular topic area, as well as exemplars with experience in improvement in the topic area. There is usually one ERP for each topic. By examining the latest available national and international evidence, guidelines and examples of improvement in the area, the role of the ERP is to:
- Identify key change principles that underlie any improvement
- Identify, where possible, successful strategies for change
- Suggest specific practical ideas for change that will generate significant improvement; and
- Suggest simple measures that would assist in assessing progress
The challenge is to determine a manageable number of potential changes that are most likely to result in improvement.
The APCC Program is not a simple ‘one size fits all solution’ but a mature approach that acknowledges the diversity that exists among general practices and health services throughout Australia. For these reasons it is important that your health service develops its own objectives, identifies improvement ideas, and generates Plan, Do, Study, Act (PDSA) cycles using the Model for Improvement to help achieve the APCC Program aims. To assist you with identifying suitable ideas for change and implementing improvements there are a range of support networks available, including the team at the Improvement Foundation.
