The key role of a Support Officer is to support health services to implement improvements across the topic areas. They would work closely with health service teams on all aspects of the Program, to enable these teams to achieve the objectives of the APCC Program. Support Officers provide dedicated advice, hands-on support and assistance throughout the Collaborative and assist General Practices in making improvements in the care they provide to patients, and in how they work as a team. They would also support the General Practice to achieve results through coaching, facilitation and support with the testing of changes using the Model for Improvement.
